Overlook Nursing is proud to announce that we have received certification from the American Heart Association (AHA) for our specialized program to help patients who have been hospitalized with heart problems avoid a return visit.

Overlook Nursing is AHA Heart Failure Certified

Our specialized, personalized program is designed to provide patients with the support and tools they need to manage their heart condition and maintain a healthy lifestyle. While the hospital treats the acute problem directly, Overlook’s program educates the patient to address the underlying causes so they can avoid re-hospitalization.

Overlook’s program combines the ability to treat existing cardiac-related conditions, assist patients post-surgery, and offer education that gives patients who have been hospitalized for heart disease or heart failure the tools and resources to avoid future stays in the hospital. We typically admit patients to get them stabilized from their immediate cardiac issues with an average length of stay at our facility of 10-14 days but this can vary. Overlook’s program also includes assessments and oversight from Overlook’s in-house cardiology team.

We also understand that after leaving the hospital, patients may require additional support to manage their heart condition. That is why we partner with in-home care agencies to ensure that patients receive ongoing care and support in the comfort of their own homes. Our team of medical professionals works closely with these agencies to ensure that patients receive the highest quality care and support.
